Advance Care Directive
This document allows the substitute decision maker to make decisions about health care, living arrangements and other personal matters on the principal’s behalf.
The interview allows for the appointment of up to 4 substitute (initial or primary) decision makers. Where 2 or more substitute decisions makers are appointed, they may act jointly, severally or jointly and severally.
Enduring Power of Attorney
This document gives the attorney the authority to manage the principal’s legal and financial affairs, including buying and selling real estate, shares and other assets, operating their bank accounts and spending money on their behalf.
The interview allows for the appointment of up to 2 initial (or primary) attorneys. Where 2 initial attorneys are appointed, they may act jointly or jointly and severally. The appointment of a substitute attorney is not permitted.
